In the article titled, “A new approach to Vancomycin utilization evaluation: A cross sectional study in intensive care unit” published in pages 279-284, issue 4, vol. 5 of Journal of Research in Pharmacy Practice [1], the sentence in the page 282 under the section “Results” is written incorrectly as “Although among three patients who experienced nephrotoxicity reaction of vancomycin only one patient received colistin concurrently.” instead of “A total of 26 (13.5%) of patients receiving vancomycin in our study, experienced nephrotoxicity reactions according to AKIN criteria. Although only two patients took nephrotoxic drug (colistin) concurrently.”
